If possible, we would appreciate you arriving to your appointments 10 minutes earlier, giving you the chance to ask any questions and to finalise paperwork before you see Dr Claudia Rodrigues.
We would also appreciate you taking the time to fill out the following forms prior to your first visit. These forms are very important as we require your medical and dental history information before commencing any treatment.
